REMINDER TO ALL ATHLETES AND PARENTS:
ALL 4 PAGES OF THE PAPER PHYSICAL ARE DUE TO THE MAIN OFFICE AT THE HIGH SCHOOL. PLEASE FILL OUT ALL 4 PAGES COMPLETELY AND EXPLAIN ANY "YES" ANSWERS ON THE HEALTH HISTORY PAGE.
PHYSICALS ARE GOOD FOR 365 DAYS IF THEY WERE APPROVED BY THE SCHOOL DOCTOR. IF THAT IS THE CASE PLEASE FILL OUT THE UPDATE FORM AND TURN INTO THE BASKET IN THE MAIN OFFICE AS WELL.
